This Nature-Based Activity Pathway is a professionally designed, printable wellbeing framework created to support consistent, person-centred, nature-based engagement with older adults across a wide range of abilities.
This product includes the pathway framework only.
Activity cards are available separately, or as part of a complete system.
The pathway is designed for use in care homes, supported living, community settings and home care, particularly where providers need to balance meaningful wellbeing support with clear, inspection-ready documentation.
What this pathway does
This pathway provides a simple, structured way to plan, deliver, record and review nature-based activities over a 12-week period, while remaining flexible enough to adapt to each individual’s needs, preferences, capacity and daily presentation.
It supports:
1:1, bedside and group activity delivery
Residents who are frail, living with dementia, bedbound, withdrawn or socially isolated
Mixed-ability settings where inclusion, dignity and emotional safety are central
It helps staff to:
Make everyday wellbeing work visible and recordable
Capture non-verbal engagement and “quiet” moments
Reflect on what works, what doesn’t, and how to adapt
Review outcomes meaningfully over time without excessive paperwork
The pathway is designed to complement existing care plans, risk assessments and activity schedules, not duplicate them.
